"Enchanted Forest" Senior Prom

*Please note this event is being held in compliance with the CA COVID-19 Guidance for Private Venues and Events and is a non-school, private event sponsored by the SVHS Parent Club.

TICKET PRICE & TERMS OF AGREEMENT

Early Bird Ticket Price:  $95 (ends May 7th)

Regular Ticket Price:    $110 (May 8th -14th) Ticket sales END on May 14th!

**IMPORTANT** When purchasing ticket the Payment info email, name and card no. (on first screen) can be a Parent, but the "Guest Info" requested on next screen should be the Attendee who the ticket is being purchased for.

Ticket Price Includes:

• Music by Professional DJ

• Taco Truck

• Beverages (Coke, Diet Coke, Sprite) and Water station
• Dessert provided at a COVID compliant station

• Voting on and Crowning of a Prom King and Queen

• Professional Prom Photos Available for Additional Charge at Event

By Purchasing a Ticket, All Attendees Agree to the Following:

• Every Attendee must have a separate ticket purchased (either ticket for SVHS Senior or a Ticket for a Guest) and the Name on the Ticket is the Attendee (not a parent).

• Attendees must show proof of ID upon entering (CA Driver's License, CA ID or High School ID)

• Attendees must provide a contact name/number for parent/guardian in the event of an emergency

• Attendees must arrive by 7pm on the night of the event or otherwise will not be permitted to enter prom 

• Attendees will not be under the influence of drugs or alcohol at any time during the event. Security will be in place during the event to monitor.

• Attendees consent to a thermal temperature and security check upon admittance to the event

• Attendees consent to a breathalyzer test upon entry and exit of the event

• Attendees will wear a mask at all times during the event except when eating or drinking

• COVID SCREENING: Attendees must complete an on-line COVID health screening & provide either proof of COVID Vaccination or Negative COVID test within 72 hours of event (verification will be administered by the independent company, CrowdPass). Please register with CrowdPass

• REFUNDS: There will be NO REFUNDS except if a student cannot attend because they believe they have COVID or have test positive for COVID -or- if we are required to cancel the event for COVID related reasons. If the case, we will provide a refund, minus any service/processing fees. 

• PARKING: Due to limited parking, attendees are highly encouraged to get dropped off/picked up or carpool

• ATTENDANCE LIMITATIONS: In the event that we are required to limit the number of attendees at the event for any reason, attendees agree and acknowledge that priority will be given first to SVHS Seniors, and then on a FIRST COME, FIRST SERVE basis based on the date of purchase of your ticket. Should we be required to limit attendance, we will provide a refund minus any service/processing fees.

• PHOTO RELEASE: A professional photographer will be taking photos throughout the event and unless otherwise notified via email, all attendees agree to grant us the right and permission to copy, use, publish, broadcast or distribute any and all photos taken during the event.

• ASSUMPTION OF COVID-19 RISK: You assume the risk that you may sustain infection, injury, or death related to COVID-19 or any other infectious disease, bacteria, virus, or other health hazard as a result of attending the 2021 Senior Prom at the Scotts Valley Four Points Sheraton (the “Premises”). You knowingly, voluntarily and expressly release and waive your rights to bring suit against the Scotts Valley Parent Club, Four Points Sheraton and any and all of its officers, directors, managers, employees, agents, volunteers or other representatives, and successors and assigns of the above mentioned parties from and against any and all actions, causes of action, suits, claims, rights, damages, losses, expenses or demands of any kind or nature whatsoever, (the “Claims”) which you can or might have as a result of, in connection with or arising out of your presence on or about the Premises, including, but not limited to, exposure to or contraction of any contagious disease which you may suffer or sustain on the Premises.

• LIMITATION OF LIABILITY: Under no circumstances will the SVHS Parent Club or the Four Points Sheraton be held responsible or liable in any way for any claims, damages, losses, expenses, costs or liabilities whatsoever (including, without limitation, any direct or indirect damages for loss of profits, business interruption or loss of information) resulting from or arising directly or indirectly from this event.
